Lead Pharmacist – Community Pharmacy & Primary Care, Cwm Taf Local Health Board, Wales
Emma Williams is the Team Leader for Community Hospital and Community Pharmacy services for Cwm Taf University Health Board and also works on a part-time basis for the NHS Wales Informatics Service (NWIS) as National Clinical Lead for ‘Choose Pharmacy’. Emma has been involved in the development of the Choose Pharmacy application since 2013, working within her Health Board role to commission services as national pathfinders and within NWIS in the design, development and implementation of the IT platform. Emma has worked as Community Pharmacy Lead for Cwm Taf since 2007; prior to this she worked as a community pharmacist.
‘Choose Pharmacy’ - a platform for innovation in community care’
The Choose Pharmacy IT platform, which underpins the delivery of a number of NHS Wales commissioned advanced and enhanced services, was developed as part of the Welsh Government commitment to improve connectivity for community pharmacy. The application has evolved over time to enable pharmacists to access patient information from other NHS clinical systems, enabling pharmacists to contribute effectively to supporting access to NHS services in line with the Prudent Healthcare principles. The presentation will outline the development of the application and its functionality, which has been designed to provide care to patients closer to home and to support the delivery of primary care services in Wales.
