Cardiovascular, Blood Pressure and Lipids - Risk Assessment.
Risk assessment tools can be used in identify patients at high risk of cardiovascular disease who can have risks reduced through life-style changes and prescribed medicines. This session will explore how interventions to improve lipid and blood pressure control can be implemented on a CCG wide scale. In particular this session will outline a population verses an individual patient approach to managing primary and secondary prevention of cardiovascular disease using the Bradford Healthy Hearts program as an example. Bradford Healthy Hearts aimed to address the problem with CVD as a main killer locally and nationally. Bradford Districts CCG has the 7th highest CVD mortality rate for under 75 year olds. The project aimed to optimise or start patients on preventive medication through a multidisciplinary approach. The roles of GPs, practice pharmacists and community pharmacists in implementation of change will be described. Results of the program will be presented. |
