Kerplunk! Remember the game with the marbles and the straws, pull out the wrong straw and all the marbles fall down and you lose the game. It’s the same with Polypharmacy. Which drugs should I stop and which ones will have my patients crashing if I pull them out. It’s a tricky problem. Our polypharmacy programme in General Practice gave the GP practice teams the skills, tools and structure they needed to engage frail patients in discussions about their medicines, sharing the goal of reducing medication burden and risk of side effects. We used behavior change psychology, quality improvement methodology and the e-PACT2 polypharmacy metrics to achieve reductions in repeat prescriptions.
