Satellite Description

There are many different models of anticoagulation service provision in primary care with standards varying widely especially where non- specialists are managing patients. Pharmacists are ideally placed to help GP practices drive standards of care up when anticoagulating their patients but there may be a reluctance to invest.

This session is designed to help pharmacists wishing to learn how they can build a case to provide a service or just get more involved in operating anticoagulation clinics in primary care. It may also appeal to those working in GP practices who could play a role in audit and review of their patient population.

Objectives include:

  • How to review the current in- house anticoagulation service to assess safety as well as potential cost savings combined with a  reduction in GP workload
  • How to review the current management of patients with atrial fibrillation to reduce risk of stroke
  • How to highlight areas where pharmacists can reduce risk and add value

There will be elements of clinical management on patients on vitamin K antagonists and DOACs.
