Hypertension is a common condition and a precursor to many others. Many regard it as “simple” to manage disease. Despite this, and the plethora of treatment options available, we are still unable to achieve adequate control in a vast number of patients. From diagnosis to treatment and ongoing monitoring - we will take a journey through cased based learning to understand the common pitfalls in the management of this “simple” condition. The session will aim to provide simple and practical advice to avoid such treatment failures, and truly optimise patient care. The session will end with an overview of an exciting hypertension service which is being rolled out in SE London. The service unlocks the potential of pharmacists and other healthcare professionals to optimise the management of patients with hypertension. | ||||
