Innovative approaches to improve medicines optimisation in CHD National and international evidence based guidelines make recommendation on how to reduce the burden and mortality from Coronary Heart Disease (CHD). However, both poor implementation of these guidelines and high levels of patient’s non-adherence to secondary prevention medicines present many missed opportunities to tackle the UK's single biggest killer (CHD). In this session, we will present the outcome of innovative approaches which were implemented at Leeds Teaching Hospitals NHS Trust to improve medicines optimisation in CHD. The award winning model, led by a consultant cardiology pharmacist, was well received by the Multidisciplinary Cardiology Team and highly praised by patients. Provisional data shows better implementation to guidelines, better adherence to secondary prevention medicines and reduction in readmission rates. |
