GP practice based pharmacists are ideally positioned to support the national drivers around Rightcare to maximize improvement opportunities in CVD areas. This satellite session will demonstrate the considerable scope a GP practice based pharmacist brings to improve prevalence by establishing robust screening, diagnosis, lifestyle modifications and medicines optimisation. This dedicated approach has shown a reduction in emergency admissions and better health outcomes. The workshop will share the methodology adopted in these systematic reviews for patients with CVD and their management plans, adhering to national and local care pathways including efficient prescribing decisions and ensuring patient safety is paramount at all times.  Maximising interoperability of GP clinical systems is another digital enhancement that has enabled the practice to review patients on Direct Oral Anticoagulants and the monitoring requirements these newer therapies warrant to be undertaken in primary care.

The satellite aims to share a real time practical approach that improves access in GP practice, delivers cost efficiency, enhances patient care and demonstrates a truly multidisciplinary approach to managing patients with CVD.
