Over the past 20 years there have been many interventions to improve outcomes for people with or at risk of CV disease, however, CVD remains the most common of premature death. Using publically available data sets, this session will investigate at how well we are delivering care in CV Disease, particularly in relation to the optimisation of medicines; and will highlight where there are opportunities for pharmacy to improve outcomes for patients, using examples from primary and secondary care. | ||||
