Reducing the risks of medicines related hospital admissions for older people living with severe frailty.

People living with severe frailty are more likely to take medicines and suffer with medicines related adverse events, including unnecessary hospital admissions and poor patient orientated outcomes. In hospital, they have higher mortality, increased length of stay and increased risk of drug related readmissions post discharge.

The session will demonstrate how pharmacists in various care settings can reduce the risks of these adverse events by leading the delivery of medicines optimisation within integrated care teams for older people. It will focus on

  • How we identify these patients, assess their needs, agree and implement a care plan to meet the needs identified, as well as co-ordinate the medicines related aspects of care.
  • How we address challenges during transition of care and the impact on re-admissions
  • How pharmacists in the different care settings can contribute and the added value of working collaboratively within and outside pharmacy
  • The key aspects of care needed to deliver medicines optimisation services fit for an ageing population
  • How we build capacity within pharmacy workforce to deliver a truly integrated medicines optimisation service
