Satellite Description

This session will be of benefit to pharmacists working cross sector or within community settings with an interest in developing new ways of working.

An increasing incidence of people with type 2 diabetes requiring insulin therapy and an aging population is leading to rising demands upon community services to administer insulin to patients who are unable to self-manage their condition.

The session describes the development of a new model of insulin administration to make better use of skill mix in providing patient care.  

The session will then look at potential opportunities for pharmacy teams in developing new roles and new models of working to support patients to remain well at home.  This includes promoting opportunities for self –administration, self -care and patient empowerment. 

It will look at examples which demonstrate the benefits of integrated working to support patients with long term conditions.
