Govan Social and Health Integration Partnership (SHIP) project is a unique service aimed to provide closer collaboration between the multi-disciplinary team (MDT) in order to provide services to patients within one of the most deprived areas in Glasgow. The project has been running since 2015 and pharmacy input was officially included in January 2018. The majority of patients included in this project had multiple complex co-morbidities as well as being vulnerable with highest rates recurring hospital admissions. During the satellite session I will aim to cover how patients were identified and medication reviews carried out. I worked closely with the surgery staff as well as district nurses, mental health, links practitioners, physio, social work and rehab and I will discuss my Interactions with the MDT and how to build on this. Results will be discussed and main learning’s from evaluation and next steps. Case studies of patients from the project will also be included for discussion. A project like this will help advice future care strategies and services. |
