Satellite Description

I will describe a non-randomised controlled pilot intervention study of patients with severe COPD who were recruited from hospital out patient clinic, then visited at home by a clinical pharmacist. They received a full medication review, any changes were agreed with the patient and the respiratory consultant,  and follow up visits were arranged as often as needed until pharmacist and patient agreed all issues had been addressed. This study offered patients tailored interventions, expedited consultant review and onward referral to other services.

I will also describe the results of this study, acknowledging its limitations, and lead a discussion about related work that could be undertaken.
