Clare:

More than 50% of patients with heart failure have chronic kidney disease and presence of kidney dysfunction worsens prognosis. This case study will discuss the cardio-renal syndrome and the complexities of managing optimisation of heart failure management in patients with kidney disease.

Lynne:

Post MI Heart Failure Management

The session will be based on a real-life case study from the post MI LVSD clinic.  It will highlight different aspects of advanced heart failure management, the role of the pharmacist independent prescriber and the role of the pharmacist in the heart failure MDT.

A presentation with opportunity for questions/debate/discussion.
