In this interactive session we will explore the medicines optimisation needs of patients with cardiovascular disease and diabetes. We will discuss new evidence in this field, missed opportunities based on real world data and way to address them. The role of pharmacy-led services in both primary and secondary care will be referred to.

Advanced level, but foundation might benefit if they want to be inspired and think about the future.
