Theme 6 is How Can The Quality Of Care Be Improved?

In March 2012, Carmel Darcy was appointed as one of the first of five Consultant Pharmacist posts within Northern Ireland (NI). Since then, she has implemented and evaluated innovative pharmacy models focusing on medicines optimisation for Older People. She currently sits as a member of the Joint Formulary Committee of the BNF, NI Regional Formulary Group, Parkinson’s Expert Reference Group, Regional Inpatient Falls Prevention Group and the NI Frailty Network. She is also involved with research in the area of dementia and medicines management. Carmel also works with education, regulatory and professional bodies to promote pharmacy development and is founder of the ‘NI Pharmacists working with Older People’ (NIPOP) network.

By the end of the presentation you will be able to:

  • describe the development, refinement and evaluation of the different Consultant Pharmacist-led models of care for Older people
  • understand the steps involved within the care-home, intermediate care and adherence pathway.
  • explain how the models were evaluated and describe their outcomes
  • describe the facilitators and barriers for regional spread.
