Satellite Description

This session will explore how a partnership was formed in Merseyside and Cheshire in order to implement electronic transfers of care for patients from Hospital to Community Pharmacy. A multidisciplinary team will discuss their experiences including the development of the strategy and business case, how implementation challenges were overcome and how Community Pharmacists were instrumental in delivering effective patient care. The session will explore how an effective evaluation of the programme is being undertaken.
