This interactive session will present a NICE recognised consultant pharmacist led medicines optimisation project which successfully re-engineered the Post-MI pathway by applying the principles of Medicines Optimisation (NICE Guideline NG5), and Medicines Adherence (NICE Clinical Guideline CG76) to improve patient experience, maximise their benefit from evidence based medicines, and reduce their cardiovascular risk factors. We will also explore missed opportunities for patients with diabetes who were admitted with MI and elaborate on the role of the pharmacist both in secondary and primary care to better optimise their medicines and risk factors. Novo Nordisk has organised and fully funded this symposium but has had no input into the content of the slides.
