This presentation will present the strains and successes of an NHS trained consultant pharmacist moving from secondary to primary care. The challenges of Medicines Optimisation in this world of problematic polypharmacy, overdiagnosis, and medicalisation is becoming ever more difficult. I will describe why I think we must move from the frantic "Mr Monkey/ Discharge Donkey" hospital pharmaceutical care model to the person centred primary care model that is so needed for people like Freda, Samir , John , Milan ....... |
