This satallite session will give you an update on developments in clinical and medicines homecare over the past 12 months (since the last PM National Forum).  It has been a busy year with the emphasis on deliver of hospital-led patient care in community settings increasing due to increasing pressure on in-patient resources and an increasing awareness that care at home is better for patient experience and outcomes.
